If you are a current patient and would like a referral for massage, print out this form and have your massage therapist complete the pertinent information and then fax, email, or hand deliver to our office.
Insurance Reimbursement Instructions
In order to get reimbursed from your insurance for out-of-network fees, you must submit the proper paperwork with your insurance identification. These are the forms needed by most insurance companies. Our office can provide you with a completed ‘SuperBill’ to submit with the Premera, Aetna, or Regence reimbursement form.

We are conveniently located at the south end of Hwy 99 (the "viaduct"), in the same business complex (South Seattle Business Park) as Arena Sports indoor soccer facility. From West Seattle take the lower bridge follow the signs to take a right on E. Marginal Way S. - then we are ~4 blocks on the left. From I-5 take the Spokane St. exit and straight along Spokane St. and take a left at E. Marginal Way S.
